Chestnut also conducts research and training throughout the U.S.A.Chestnut Health Systems is seeking a full-time Community Support Specialist for our Community Support Services program providing services to adults experiencing chronic mental health symptoms and/or co-occurring symptoms in St. Claire County, IL location.**Please note to be considered for a position you must attach a resume, complete the application, and answer all screening questions.ResponsibilitiesEducating and building skills of daily living (i.e. including hygiene, grocery shopping, laundry)Providing for referral, linkage and advocacy to recommended community resources especially involving hospitals, state & federal aid agencies, court systems, physician offices, food pantries, landlords and local churches/charitiesAssistance trying to locate housing and or shelterIdentification of personal recovery/develop personal recovery planSupport and advocacyAssistance linking to resources in communityAssistance linking to primary or specialized careAssistance with public entitlements (i.e. SSA, Medicaid)Medication education and monitoringSkill building in the natural setting (70% of services are done in client s home/community)Crisis intervention and education on how to manage crisis situationsEducation on psychiatric diagnosis and treatment optionsDiscussion of stigmaFamily support and educationCommunity-based therapyRepresentative payee shipQualificationsMust have a bachelor s degree and five (5) years supervised mental health experience; or a master s degree in counseling, psychology, social work or related field with less than 1,000 hours of supervised mental health experience.Must Have a valid driver s license, private automobile insurance and be insurable.Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).Must possess clinical and case management skills, work from a team approach, have good time management, good communication and organizational skills, be able to display positive interactions with colleagues and other community representatives.Must be supportive of working in a recovery-oriented environment.
